1- Declare an ESS event

To report a safe event, click on the button"Register an SSE event".

It is also possible to open a secure event recording directly by clicking on the shortcut button on the Software home page.

-Section 'General Information' Section:


Complete the different fields on the event reporting page:
Fields marked with a * must be entered.

-Internal No.= Automatic generation of a security event reference
-Site= Select the site concerned by the security event being created.
-Service= Word of service associated with the event (associated with the parameter 'Service' parameter list)
-Date of event / Time
-Venue: If internal = sector field (associated with the parametric list 'Sector') / If External = free fields)
-Category= Allows you to select one of 5 categories: Accident / Incident / Almost accident / Dangerous situation / Dangerous behaviour.
-Consequences= Allows you to select one or more consequence(s): Body injury(s) / Material Damage / Pollution.
- Sector: Allows you to locate the event (associated with the parameter list 'Sector').

-Section 'Description':This section is composed of a succession of free fields to detail the identified SES event:

- Description of the SES event = mandatory field (taken in CERFA) to briefly describe the event.

- What happened BEFORE / COMPANY / AFTER the event = non-mandatory fields that can assist with event analysis.

- Immediate action(s)

- Cause according to the employee

- PJ import button = allows you to attach documents that will be included in the appendix to the SES event PDF.


-Person(s) concerned:This section provides information to the victim (if accident with bodily injury) and/or those involved in the SES event.

The victim may be:
- An employee of the company. In this case, it is possible to elect:
  1. The employee's name/first name (related to the employee database)
  2. The position of the employee:
â-a If the HR module is active on the account: select the position in the 'Publisher' field (field linked to the HR module position register)
â-a If the HR module is inactive on the account: select the position in the 'Place Occupied' field (field attached to the parameter list 'Place occupied' to be completed in advance)
- A person outside the company: Generation of the same fields to be entered manually.

-relevant material(s):This section identifies a material related to the reported SES event. There are two cases:

- The relevant material is owned by the company: the scope of the equipment is a list related to the material database.
- The relevant material does not belong to the company (external): management of a free field

-Kind(s) or first person notified:This section identifies the event(s) related to the event or first informed person(s)

-Third parties:This section identifies the third(s):
- If no third party (by default): no field to fill in
- If third party: management of free fields to identify the third party and his insurance organization.
NB: these data will be included in the CERFA for the model reporting

-Validation of the declaration on Symalean:

To validate the declaration of an SES event, it is necessary to design at least 1 recipient in the field at the bottom of the form.

One or more recipients may be set by default via the parametric list'Declaration Recipient'present in the PARAMETRAGE module.
- If the HR module is active on the account: the recipient of the event may be associated with the 'Responsible' of the designated employee as a victim.
- If inactive HR module on the account: The recipient can be associated with one or more users to be defined in the parametric list.

After completing the accident report form, click button"Validate"

You then return to the SES event register on which the record appears below the state"Declared"
