Several states are possible for an effectiveness assessment:

Planning
The evaluation is planned and verification deadline has not yet passed
Late
The evaluation is planned and the period of verification is exceeded
Effective
The evaluation is conducted and the training was found to be effective for the affected employee. Evaluation matrix score > or equal to 50%
Ineffective
The evaluation is conducted and the training was found to be ineffective for the employee concerned. Evaluation matrix score < to 50%
